Inspector Notes

A renewal inspection was conducted on March 7, 2024, from 4:30pm to approximately 5:45pm. There were 14 children present and two staff members. The Inspector reviewed six children?s records and two staff records. The Inspector observed the interaction of staff and children throughout the inspection. Children were observed during group time and free play prior to being picked up by their parents. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the director at the exit interview.

Standard 8VAC20-780-160-A
REPEAT VIOLATION. Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.
Evidence:
Staff #1 has a TB screening dated 8/25/2019, which is expired and not within two years from the date of the inspection and date of their initial screening.
Plan of Correction: Staff #1 TB screening will be completed by a licensed physician as well updated and placed in file.
Standard 8VAC20-780-350-C
Based on observation and interview, it was determined that the center did not ensure that when children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
Evidence:
1. There were 31 children present at the center with two staff members. The youngest child in the group is 3 years old in which the ratio would be one staff member to ten children. The center would require two additional staff members to be in ratio.
2. Staff #1, Director, confirmed that the youngest child in the center is 3 years old which would put the required ratio of 1:10. Staff #1 confirmed that they had 11 children over.
Plan of Correction: Additional staff & floaters are currently in the hiring process to support our 3-year old ratio.
