Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated on 01/21/2022 and concluded on 02/07/2022. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 2 children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a record review the center did not ensure all staff had current tuberculosis screenings at least every two years rom the date of the first initial screening or testing.
Staff #3's (hired 11/15/19) last tuberculosis screening was dated 11/07/2019
Plan of Correction: Staff #3 will obtain a screening/test this week.

Based on a record review the center did not have annual approval from the health department.
The last documented health inspection was dated 09/25/2022.
Plan of Correction: The health inspection is handled by the school system. The district manager is working on obtaining a current inspection.
