The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1 did not have a copy of her immunization records and has been enrolled at the center for over one month.
Plan of Correction: The center responded with the following: The center was able to obtain a copy of the child?s immunization records that same day of the inspection. The record was placed in the child?s record and sent to the inspector.
