During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on April 29, 2025. The on-site inspection began at 4:00 pm and ended at 5:00 pm. The inspector reviewed compliance in the areas listed above. There were 80 children present and 7 staff. The inspector reviewed 8 children?s records on-site and 7 staff records electronically on April 40, 2025. This inspection included observations, interviews, document review, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The program is required to submit to VDH and implement a plan to test potable water for lead.

The program did not have evidence of a testing plan submitted to VDH and the program did not have evidence of the implementation of the required testing plan.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center's medication policy states that " All leftover or expired medication will be given back to the child?s parent for disposal. Medication not picked up by the parent within one month will be flushed down the toilet or disposed of in a garbage container that is not accessible to children?.

Child # 9's emergency medication expired in September 2024 and had not yet been returned to the parents.
