Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 09/21/21 and concluded on 09/28/21. The site director was contacted by telephone and a virtual inspection was conducted. There were 86 children present with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: 1) The record for Child #4, enrolled on 09/08/21, did not contain immunizations prior to the child's first day of attendance. The immunizations were received by the program on 09/28/21. 2) During interview, a member of management reported the immunizations were not submitted to the program prior to the child's first day of attendance.

Evidence: 1) The most recent TB screening in the record for Staff #3 was dated 06/03/2019. The TB screening expired on 06/03/2021. 2) During interview, a member of management confirmed this is the most recent TB screening on file for Staff #3. Staff should resubmit TB test results at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.
Evidence: 1) The record for Staff #1, Staff #2, Staff #3, Staff #4 and Staff #5 were reviewed. The records did not contain documentation that the staff members have completed daily health observation training within the last three years. 2) During interview, a member of management reported there are no staff members on duty that have current daily health observation training. Staff #6 last completed daily health observation training on 08/22/18. The daily health observation training for Staff #6 expired on 08/22/2021.
Evidence: 1) The licensing inspector reviewed the center's emergency drill record. There was no evacuation drill documented for June 2021. 2) During interview, a member of management confirmed that a monthly practice evacuation drill was not conducted in June 2021.
