Inspector Notes

A monitoring inspection was initiated and concluded on 5/26/2023 There were 44 children present with 8 staff supervising.The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total 5 child records and 5 staff records were reviewed. The inspection started at 3:15 pm and concluded at 5:10 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

* REPEAT VIOLATION*
Based on a review of records and interview, the licensee did not always ensure that the results of a Federal Fingerprint based Criminal Record Search are obtained prior to employment or volunteer service.
Evidence:
Five staff records were reviewed.
The record for staff #5, whose date of employment was documented as 4/25/2023, did not contain the results of a Criminal History Search.
Staff #5 was working alone in a classroom with 7 children ages 5-7 years old at the time of the inspection.
Staff #6, who was in charge of the center at the time of the inspection, confirmed that this documentation was not present in staff #5's record.
Plan of Correction: The Program Director will follow up to see if the Criminal History Report is being processed by the Office of Background Investigations (OBI) and document the correspondence. If OBI has not received the fingerprints for processing, then a new appointment for fingerprints will be set.. A copy of the correspondence or a copy of the new appointment confirmation will be forwarded to the inspector within 10 days.
