Inspector Notes
An unannounced monitoring inspection was conducted on 11/16/2023 from 3:30 pm to 5:40 pm. At the time of entrance, 48 children, ages 11 weeks to 7 years old were in care with 11 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed eating an afternoon snack, playing with toys and manipulatives, dancing to music. Infants were observed sleeping in cribs, sitting in bouncy seats, crawling and exploring toys, and being bottle fed. Diaper changing and hand washing were observed in the infant room. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program Repeat violations were found and are identified within this report.
Violations
2Based on a review of records, the licensee did not always ensure that each staff member submits a negative tuberculosis screening prior to having contact with the children in care.
Evidence:
Five staff records were reviewed. The record for staff # 2 did not contain the result s of a Tuberculosis screening. Staff #2 began employment as a teacher's assistant, working in the classroom on 11/8/2023.
The program director confirmed that she did not have documentation of a Tuberculosis screening for staff #2.
Based on observation, the licensee did not ensure that all areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions include, but are not limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence:
In the three year old classroom, there were two, one foot long sections of bottom trim on the walls, which were pulled away from the wall and surrounded by peeling paint.
In the three year old classroom, there was a one foot square of carpet which had come unglued and separated from the rest of the carpet around it posing a trip hazard.
On the outdoor play space, there were two aluminum vent surrounds which had sharp corners and sharp edges. The vent surrounds were located on the exterior wall of the building at a height accessible to children in care.
The facility staff will inspect classrooms and play areas daily and report issues to the program director for repair.
The program director will inspect areas that children access at least weekly to ensure items needing attention are fixed before children have access to them.
