Inspector Notes
An unannounced, on-site monitoring inspection was completed on November 4, 2024. The on-site inspection began at 12:40 pm and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. There were 57 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 14, 2024. A POC submitted after this date will not appear on the public website.
Standard 22.1-289.035-B-2
(Repeat violation) The center must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #4, who had been employed for ten months, did not have a completed national criminal background check. A copy of the results from staff #4's previous employment, dated 9/22/2023, were in the record. The program Director confirmed that the Criminal Record Results were from staff #4's previous employer and that a new Criminal Record Check had not been obtained for her current employment at this program.

Standard 8VAC20-780-260-B
The center shall have annual approval from the health department. The most recent health inspection report available for review was dated April 24, 2023 which is more than a year old.

During the designated rest period, there shall be 1 staff per 10 children that are 16 months through 24 months old. Staff #1 was observed supervising 12 children (youngest 18 months old) napping in the toddler classroom.

The diapering area shall have the appropriate disposal container that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the container. The diaper disposal container in the children's bathroom by the diapering table would not open with the step.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment and each staff record is to have an emergency contact. Staff #5, who has been actively working at the center for the past 2 months, did not have a second reference check and a emergency contact with name, address and telephone number.
