During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on September 8, 2025. The on-site inspection began at 12:45 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were forty children present and 6 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-270-A
(Repeat violation) Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. There was peeling paint on a stall door in the hall bathroom. There are three missing wood panels in the fence on the playground leaving exposed rusted nails.
Plan of Correction: The staff door will be repainted and covered. The boards in the fence will be replaced.
Standard 8VAC20-780-350-B-1
The required ratio for children age birth to sixteen month is one staff per four children. One staff was supervising nine children in the toddler room at 1:00 pm. The youngest child was fourteen months old.
Plan of Correction: A written assessment will be completed and signed by the director, lead teacher and parent assigning the child to the toddler room.
Standard 8VAC20-780-570-A
When children are placed in infant seats in a feeding table, the protective belt shall be fastened securely. An infant was sitting in a feeding table and the belt was not fastened. The belts had been removed to be washed and had not been placed back in the seats.
Plan of Correction: The safety straps will be put back in the seats.
Standard 8VAC20-780-570-E
Prepared infant formula must be labeled with the child's name and date. Three bottles in the infant room refrigerator were not labeled.
Plan of Correction: A reminder will be sent out to parents to label the bottles and staff will monitor to ensure it is done.
