Inspector Notes

A monitoring inspection was conducted on November 15, 2023 from 10:19 am until 12:48 pm at the program. During the inspection, there were 135 children in care and 34 staff supervising including administrative staff. The inspector completed observation of the children in care engaged in active play at the outdoor playground, presenting turkey disguise assignments to peers and staff, asleep in cribs, completing bathroom breaks, preparing for lunch, working on classroom activities, and dancing to interactive music. The sample size was 5 staff records. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.

Based on review of staff record and interview, the provider did not ensure each person in a supervisory position has been certified by a practicing physician or physician assistant.

Evidence: In Staff #1's and Staff2's records (start date 11/13/2023), there were no staff health reports found for review. Interview with staff confirmed the documentation or the staff health report was not available during the inspection.
Plan of Correction: The program director will have staff complete all documentation for submission today to be completed by Friday.

Based on review of staff records and interview, the provider did not ensure to obtain a copy of results of a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.

Evidence: In Staff#3's record, there was no out of state results found for review during the inspection. Interview with staff confirmed the results were not available for review.
Plan of Correction: The program director will have staff complete all documentation for submission today.
