Inspector Notes

An unannounced investigation was conducted today in response to a complaint regarding the physical plant of the facility. During the investigation staff were interviewed, the files of children and staff were reviewed, and classrooms were observed. The preponderance of evidence does not support the allegations. This complaint has been determined to be not valid. The exit interview was conducted with the director. A monitoring inspection was also complete and areas of non-compliance are identified in this report. Based on observation, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

Based on observation, the areas and equipment of the facility were not kept in a clean safe and operable condition.

1) The gate in the "Rainbow Fish" room was loose.
2) The restroom in the "Rainbow Fish" room displayed an unclean sink, toilet and floor.

Based on observation, electrical outlets did not have protective covers that are of a size that cannot be swallowed by children.
