Based on observation, the provider failed to ensure that the vehicle used to transport children meets the safety standards set by the Virginia Department of Motor Vehicles;
Evidence:
The van used to transport children and in use today has an expired safety inspection. The sticker on the windshield is for 3/2020.

Based on caregiver statement, the caregiver left alone with children does not speak English.
Evidence:
At Licensing Inspector arrival (3:25PM) there was one caregiver alone with children. The caregiver stated she did not understand English. (Caregiver C)

Based on observation, the home, inside and out, has not been maintained in a clean safe and operable condition.
Evidence:
The backyard area contains building materials such as deteriorating lumber stacked by the fence, a spray can of WD-40, an unlocked and open storage shed that contains spray paint cans, and building materials accessible to children. Although children were not in the backyard during today's inspection, these conditions have existed for more than today's date.

Based on observation, there is no at non-climbable barrier at least four feet high, separating the above ground pool from the backyard play area.

The above ground pool installed in the backyard, does not have a non-climbable barrier preventing access to the pool. The chain-link fence gate is open and unlocked, there is no gate or fence preventing access to the pool from the backyard.

Based on observation, all animal excrement has not been removed promptly and disposed of properly.
Evidence:
Animal excrement has not been removed from the backyard. There are multiple piles of dog excrement throughout the backyard.

Based on a review of records and provider statement, daily attendance has not been kept.

The attendance sheet was not completed for Friday 2/10/2023.
