22VAC40-185 STAFFING AND SUPERVISION.

Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 03/23/2021. A self-reported incident was received by the department regarding allegations in the areas of supervision. The director was contacted by telephone to conduct the investigation.

The evidence gathered during the investigation supported the self-report of non-compliance with (1) standard and can be found on the Violation Notice issued to the facility.

Standard 22VAC40-185-340-F
Based on interview, a child under 10 years of age was not always within actual sight and sound supervision of staff.

Evidence -
1. On the date of inspection (03/23/2021), the center self-reported to the Division of Licensing that Child #1 (3 years of age) was left unsupervised for a total of 2 minutes on 03/19/2021 at approximately 4 pm.
2. The Director stated (during an interview conducted on the date of inspection) that Staff #1 failed to conduct a name to face roll call as the children in the Piglet Room transitioned from their classroom to the playground; leaving Child #1 alone in the classroom.
3. The Director found Child #1 (sitting on the room's carpet) while conducting a new staff tour and concluded that 2 minutes had past since Staff #1 left with the other children to the playground.
4. Including Child #1, there were a total of 5 children in the Piglet Room prior to Staff #1 taking the class to the playground at 4 pm.
Plan of Correction: Staff #1 was terminated from employment. The Parents of Child #1 were notified of this incident involving their child.
