Inspector Notes
A self-reported incident was received by the department on 04/06/2023 regarding supervision. An investigation in response to the self-reported incident was initiated on 04/10/2023 and concluded on 05/09/2023. An on-site investigation was conducted on 04/10/2023 between the hours of approximately 10:15 a.m. and 11:35 a.m. Staff interviews were conducted, video footage observed and records were reviewed. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were documented on the violation notice issued to the program. Violations
3Evidence:
1. On 04/06/2023, at approximately 9:29 a.m., the video recording shows Staff #2 walking Child #1 (age: 3 years) towards the bathroom on the School Age playground.
2. At approximately 9:30 a.m., the video recording shows Staff #2 going back to the playground area where the rest of her students were playing, without Child #1.
3. At approximately 9:47 a.m., the video recording shows Staff #2 and Staff #3 going into the building with their class.
4. Staff #1 stated that when they went to the playground at approximately 9:50 a.m., they were alerted that Child #1 was in the bathroom.
5. When Staff #1 went to retrieve Child #1 from the bathroom, the child was still sitting on the toilet and did not appear to be crying.
6. Child #1 was in the bathroom, unsupervised, from approximately 9:29 a.m. to approximately 9:50 a.m.
7. Staff #2 and Staff #3 did not realize that Child #1 was not with the class.
Evidence:

2. At approximately 9:30 a.m., the video recording shows Staff #2 going back to the playground area where the rest of her students were playing, without Child #1.
3. At approximately 9:47 a.m., the video recording shows Staff #2 and Staff #3 going into the building with their class.
4. Staff #3 stated that they were not aware that Staff #2 had taken Child #1 to the bathroom.
5. Staff #1 stated that when they went to the playground at approximately 9:50 a.m., they were alerted by Staff #4 that Child #1 was in the bathroom.
6. When Staff #1 went to retrieve Child #1 from the bathroom, the child was still sitting on the toilet and did not appear to be crying.
7. Child #1 was in the bathroom, unsupervised and with the door closed, from approximately 9:29 a.m. to approximately 9:50 a.m.
Evidence:
1. The center has a Name-to-Face policy that states that the Name to Face checks should be conducted ?before and after every transition from place to place.? It also states that Name to Face checks should be conducted ?before you leave the playground and again when you get back to the classroom.?
2. On 04/06/2023 Staff #1 stated that the Name to Face check was not conducted by Staff #2 or Staff #3 before leaving the playground or when they returned to the classroom.
3. Staff #2 signed a copy of the Name-to-Face policy on 03/06/2019.
4. Staff #3 signed a copy of the Name-to-Face policy on 09/05/2022.
5. Staff #1 stated that they reviewed the Name-to-Face policy with all center staff at new hire orientation and during staff meetings.
