Inspector Notes

An unannounced Monitoring Inspection took place on September 5, 2023 between the hours of approximately 12:50 p.m. and 2:25 p.m. There were 4 classrooms observed with a total of 40 children with 5 staff within the supervision guidelines. The children were observed napping and sitting on their cots. A complete inspection of the physical plant, children and staff records, fire drill log, and medication were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center failed to obtain documentation that each child has received required immunizations before the child can attend the center.

REPEAT VIOLATION

Based on record review, the center failed to obtain a physical examination for each child before the child?s attendance or within 30 days of the first day of attendance.

Evidence: Child #2 (start date: 07/10/2023) did not have documentation of a physical examination on file.
Plan of Correction: Parent scheduled a visit w/ new pediatrician.

Based on record review, the center failed to ensure that all staff complete the Virginia Department of Education-sponsored orientation course within 90 days of employment.

Evidence: Staff #5 (start date: 05/10/2023) did not have documentation of completion of the Virginia Department of Education-sponsored orientation course on file.
Plan of Correction: Staff is working on completing it within a week.
