Inspector Notes

An unannounced renewal inspection was conducted on April 5, 2024 between the hours of 9:25am - 1:00pm. There were 79 children directly supervised by 11 staff. The physical plant, 5 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, coloring, singing songs and playing outside. There was an assortment of age appropriate materials for the children in care. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-160-A
Based on record review and interview, the center failed to ensure that each staff member submits documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence:
1) Staff #2 (DOH: 11/29/2023) record did not contain documentation of negative tuberculosis screening.
2) Staff #5 confirmed that Staff #2 did not have documentation of a negative tuberculosis screening.
Plan of Correction: The Staff #2 was sent for another TB test as the current one says chest clear only,
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) In the Happy Hippos classroom, 2 open containers of cleaning liquids were observed on a high shelf above the rocking chair.
2) In the Lively Ladybugs classroom, 1 bottle of cleaning spray was observed in an unlocked upper cabinet above the sink.
3) In the Eagles classroom, 1 bottle of cleaning spray and 1 container of cleaning wipes were observed in an unlocked lower cabinet under the sink.
Plan of Correction: 1) The sanitizing containers were removed from the shelf same day & are relocated away from children & diapers since this is a NAEYC requirement.
2) The cabinet was unreachable to children & we locked it the same day.
3) The cabinet was locked on the spot.
Standard 8VAC20-780-580-B
Based on observation and interview, the center did not ensure that any vehicle used by the center for the transportation of children shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of children.
Evidence:
1) Two of the three transportation buses have an expired Virginia State Inspection dated March 2024.
2) Staff #5 confirmed the Virginia State Inspection is expired and is actively being used for transportation of children.
Plan of Correction: The buses are re-inspected and up to date now.
