Inspector Notes

A complaint was received by the licensing office on June 5, 2024, in regard to children's records and reports. An investigation was initiated on June 11, 2024 between the hours of 12:50pm - 2:20pm, and concluded on June 12, 2024. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Areas of noncompliance can be found in the violation notice. Please submit your plan of correction within five business days in order for it to be included on the public website. Standard 8VAC20-780-50-B
Based on interview, records on children were not made accessible for two years after termination of services.
Evidence:
1. Staff #1 and Staff #2 stated that the record for Child #1 (last date of enrollment: 05/23/2024) was not available for review at the time of inspection.
Plan of Correction: The child documents were sent by email after the inspection. We will ensure that all files are organized and immediately available upon request.
Standard 8VAC20-780-80-B-2
Based on record review and interview, the center did not inform the superintendent?s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurred while the child was under the supervision of the center and required outside medical attention.
Evidence:
1. The centers accident/injury report stated that on 05/14/2024, Child # 1 (age: 2 years) was "opening and closing the potty door until they closed it on their finger. Their nail is displaced."
2. Staff #1 stated that on 05/15/2024 they were notified by the parent that Child #1 went to the Emergency Room and was diagnosed with a broken finger.
3. There was no record of the injury that occurred to Child #1 on 05/14/2024 that required outside medical attention, or that the injury was reported to the superintendent's representative.
4. Staff #1 stated that they were unaware of the requirements for reporting injuries of a child that occur while the child is under the supervision of the center and required outside medical attention.
Plan of Correction: As soon as we are made aware of the procedure, we immediately followed it and reported the details to licensing on the same day of inspection, June 11th?2024. We will continue to follow this procedure when required.
