Inspector Notes
A complaint was received by the licensing office on June 5, 2024, in regard to children's records and reports. An investigation was initiated on June 11, 2024 between the hours of 12:50pm - 2:20pm, and concluded on June 12, 2024. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Areas of noncompliance can be found in the violation notice. Please submit your plan of correction within five business days in order for it to be included on the public website. Violations
2Evidence:
1. Staff #1 and Staff #2 stated that the record for Child #1 (last date of enrollment: 05/23/2024) was not available for review at the time of inspection.
Evidence:
1. The centers accident/injury report stated that on 05/14/2024, Child # 1 (age: 2 years) was "opening and closing the potty door until they closed it on their finger. Their nail is displaced."
2. Staff #1 stated that on 05/15/2024 they were notified by the parent that Child #1 went to the Emergency Room and was diagnosed with a broken finger.
3. There was no record of the injury that occurred to Child #1 on 05/14/2024 that required outside medical attention, or that the injury was reported to the superintendent's representative.
4. Staff #1 stated that they were unaware of the requirements for reporting injuries of a child that occur while the child is under the supervision of the center and required outside medical attention.
