During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on March 26, 2025, and completed on April 15, 2025, in response to a self-report received by the licensing office on March 24, 2025, relating to Staffing and Supervision. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 22, 2025. A POC submitted after this date will not appear on the public website.

Repeat Violation

An applicant for employment that has received qualifying results on the fingerprint-based background check and has requested an out of state central registry background check, may work in the child day center prior to receiving the results, provided that the applicant is supervised at all times by a person who received a qualifying result back on a background check conducted in the past five years.

Staff #1, who has been employed for over two weeks, was working alone with a group of children (all age: 3 years) prior to the center receiving qualifying results for staff #1s out of state central registry background check from one state that was requested three weeks ago.
Plan of Correction: We will adjust our staffing schedule to make sure that the staff is not by themselves until we receive the complete background check results.
Standard 8VAC20-780-230-A
Individuals from independent contractors shall not be counted in the staff-to-children ratios unless they meet the qualifications for the applicable position.

Staff #1 walked out of a classroom leaving independent contractor #1, who did not meet staff qualifications and was not an employee of the center, alone with children for approximately nine minutes.
Plan of Correction: Our teachers are aware that any independent contractor will not count
towards the child teacher ratio. We have reminded all our current staff again.
We are adding a policy regarding third party contractor/therapist that will be
visiting on site. This policy will be a part of our on boarding guide. In addition to
this, we have provided contractors/therapist with a visitor tag for more clarity
that they are not maintaining the child teacher ratio.

On 03/24/2025, staff #1 left a classroom with children (all 3 years of age) out of sight supervision, for approximately nine minutes with independent contractor #1 who did not meet staff qualifications and was not an employee of the center.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 03/24/2025, staff #1 did not ensure the care, protection, and guidance of a classroom of children (all 3 years of age) when she walked out of the classroom leaving them alone with independent contractor #1, that was not an employee of the center, for approximately nine minutes.

Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) and first aid as appropriate to the age of children in care. The training shall include an in-person competency demonstration.

Staff #1, who had been employed for over two weeks, was working alone with a group of children (all age: 3 years), and did not have a current certification in CPR and first aid that included an in-person competency demonstration.
Plan of Correction: Moving forward, all the new hires will complete the in person competency demonstration for CPR/First Aid training.
