During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on May 23, 2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to Independent Contractors; Volunteers, Supervision of children, First Aid Training and Cardiopulmonary Resuscitation and Background Checks.The on-site inspection began at 12:40pm and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and 9 staff. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 5, 2025. A POC submitted after this date will not appear on the public website.

Cots, mats and beds used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

In classroom #1, the cots used by child #1, #2, #3 and #4 (all age: 5 years), did not have a bottom cover.
Plan of Correction: Reminder was sent to the parents and the sheets were brought the next day. We are keeping a close eye for cot sheets in classrooms.
