Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 8, 2021 and concluded on April 9, 2021. The director was contacted by telephone to initiate the inspection. There were nine children present with one staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined one non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-185-140-A
Based on a review of records for children, an interview and written communication with administrative staff, the school failed to ensure all children had physical examination records in their files no later than one month after initial attendance. Evidence: Child 2 began on October 8, 2020 and there was not a physical exam record in the file. Only the immunization record was found in the file. Administrative staff indicated that the record could not be located and if it was sent by email, it was deleted.
Plan of Correction: The head of school contacted the parent who arranged for a FAX of the physical examination information and submitted the information to the school. The required record was obtained for the school file and shared with the licensing inspector. The administrative staff will make sure all required health records are obtained on time.
