Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 9, 2022 from 11:30 a.m. through 12:30 p.m. Upon arrival, there were 10 students ages 4 through 5, in the care of 1 staff. Four children records and two staff records were reviewed and updated. Areas of non-compliance may be found on the violation notice.

Based on observation the center failed to have one hazardous substance in a locked place.

Evidence:

1. One can of Lysol Spray was observed on a counter in the preschool bathroom.
2. Staff confirmed the Lysol Spray was not in a locked place.
Plan of Correction: Staff member immediately removed the Lysol Spray and placed in a locked area.
