Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 4:29/2025. The inspection started at 11:35am and concluded at 12:45pm. There were three children present and one caregivers; the point total was ten. The inspector reviewed three children?s records and four caregiver/household members records on site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
