Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 9:15 am through 10:30 am. There were a total of nine children (22points) observed with two caregivers. A total of five children's files and two staff/household member's files were reviewed. The children were engaged in the following: toy play, breakfast, circle time, bathroom breaks, hand washing, outside time and other organized activities. Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4's record (Start date-07/20/2022) did not contain documentation of a physical examination.
Plan of Correction: The provider will ask the parents for a copy of the physical examination and document the record.
Standard 8VAC20-800-60-B
Based on record review, children's records did not contain required information.
Evidence:
1. Child #4's record (Start date-07/20/2022) did not contain documentation of the medical insurance policy number and written authorization for emergency medical care should an emergency occur.
2. Child #5's record (Start date-10/11/2022) was missing documentation of one name, address and telephone number of an emergency contact.
Plan of Correction: The provider will get the missing information from the parents and document the record.
Standard 8VAC20-800-70-A
Based on record review, the parents of one child were not provided the required written information before the child's first day of attendance.
Evidence:
1. Child #5's record (Start date-10/11/2022) was missing medication administration policies and the provisions of the family day home's emergency preparedness and response plan.
Plan of Correction: The provider will give the parents a copy of the provisions of the Emergency Plan and policies for administration of medication and have them to review and sign the document. The record will be documented.
Standard 8VAC20-800-920
Based on observation, when meals or snacks are provided by the family day home, the menu for the current one-week period was not dated.
Evidence:
1. The posted menu on the parent board was not dated for the current week.
Plan of Correction: The provider will date the menu for the current week.
