Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:30am. There were a total of 4 children (11 points) observed with two caregivers. A total of four children's files and two staff/household member's files were reviewed. The children were engaged in the following: toy play, breakfast, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Based on record review, children's records did not contain the required information.
Evidence:
1. Child #2's record (Start date-02/23/2022) did not contain documentation of the annual review of the child's emergency contact information.
2. Child #4's record (Start date-06/01/2021) did not contain documentation of the annual review of the child's emergency contact information.
Plan of Correction: The provider will have the parents to review the emergency contact information and document the record.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-10//11/2022) did not contain documentation of the Policies of the administration of medications, Provisions of the Emergency Plan, and the Liability Insurance Declaration form.
Plan of Correction: The provider will have the parents to sign the policies for administration of medication form, Provisions of the Emergency Plan form, and the Liability Insurance Declaration form today.
