Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:30am through 11:30 am There were a total of seven children (15 points) observed with two caregivers. A total of six children's files and two staff/household member's files were reviewed. The children were engaged in the following: toy play, games, circle time, and other organized activities. 5Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1's record (Start date-09/05/2023) did not have documentation of a physical examination.
2. Caregiver #1 confirmed that Child #1 did not have a physical examination.
Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-09/05/2023) did not contain two emergency contacts addresses.
2. Child #6's record (Start date-07/20/2022) did not contain documentation of the review of the child's emergency contact information.
Evidence:
1. The emergency preparedness plan for the family day home was not reviewed for the current year. The last review was documented in writing on 04/26/2022.
Evidence:
1. Child #1's record (Start date-09/05/2023) did not contain documentation of immunization records.
2. Caregiver #1 confirmed that Child #1 did not have immunization records before the first day of attendance.
