Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on May 1, 2025, as a part of the licensure period.The on-site inspection began at 9:15am and ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were three children present and two caregivers; the point total was 6. The inspector reviewed six children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation

The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

Child #4 did not have additional immunizations once every six months. Documentation of the last immunization record was dated 07/03/2023 when the child was 16 months old. Child #4 has been in care for 21 months.
Plan of Correction: The provider will ask the parents to provide a copy of the current immunization record for Child #4. Moving forward, the provider will ensure that children under the age of two years old have immunization records up to date every six months.
