Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 16, 2025, as a part of the licensure period. The on-site inspection began at 10:25am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were eight children present and two caregivers; the point total was 17. The inspector reviewed six children?s records and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-140-D
A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.

The substitute provider did not record and sign the time of arrivals and departures for each day that they worked.
Plan of Correction: The substitute provider will complete the department's model form to document the arrival and departure times of the days worked.
Standard 8VAC20-800-60-B
Children's records shall contain the policy number of the medical insurance.

The record for Child #4 did not contain the policy number of the medical insurance.
Plan of Correction: The provider will obtain the policy number of Child #4's medical insurance today and document the record.

Repeat Violation

The family day home shall obtain documentation of additional immunizations for the child who is not exempt from the immunization requirements once every six months for children under the age of two.

Immunization records for Child #4, 18 months old, did not contain additional immunizations once every six months. Documentation of the last immunization was taken when Child #4 was 10 months old and nine months ago.
Plan of Correction: The provider will contact the parent of Child #4 to obtain current immunization records and document the record. Moving forward, the provider will check the records monthly to ensure up to date immunization records for children.

When meals or snacks are provided by the family day home, them menu for the current one-week period shall be dated.

The menu for the current one-week period was not dated.
Plan of Correction: The menu was dated for the current week at the time of the inspection. Moving forward, the provider will date the menu each week.
