Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on April 6, 2026, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 10:40am . The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 12. The inspector reviewed seven children?s records and two caregiver/household member(s) records on-site.This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers shall obtain current certification in cardiopulmonary resuscitation(CPR) to he age of the children in care every two years.

Caregiver #1 did not obtain current certification in CPR. Caregiver #1's certification expired on 01/25/2026.
Plan of Correction: The provider will obtain current certification in CPR this week.

Repeat Violation

Children's records shall contain the policy number of the child's medical insurance.

The record for Child #1, in care for two months, did not contain the policy number of the child's medical insurance. The record for Child #7, in care for five months, did not contain the policy number of the child's medical insurance.
Plan of Correction: The provider obtained the policy number of the Child #1's medical insurance at the time of the inspection and will obtain the policy number of Child #7's medical insurance today. The provider will ensure to check over children's records carefully to ensure that paperwork is complete.

Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the identity and age of the child.

Verification of the identity and age of Child #7, in care for five months, was not obtained from the parent. Caregiver #1 confirmed that verification of the identity and age of Child #7 was not obtained.
Plan of Correction: The provider will obtain verification of identity and age of Child #7 today and document the record.
