Inspector Notes

A monitoring inspection was completed from 12:10 pm until 1:20 at the center and then from 1:20 until 2:20 at the main office where staff records were reviewed. There were 11 children in care with five children supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center did not have a physical examination for a child.
1. Child #4 was enrolled on 08/15/22 and there was not physical available; staff confirmed there was not physical exam in the file.
Plan of Correction: The parent was called and will be brining in a physical for the child

Based on document review and interview, the center failed to have documentation of an emergency evacuation drill.
Evidence:
1. There was no documentation for 03/2023; staff verified there was no documentation.
Plan of Correction: Center staff will document each emergency drill in the future.
