Inspector Notes

An unannounced on-site inspection was initiated and completed on 5/7/25. The on-site inspection began at 10:30 and ended at 11:40. The inspector reviewed compliance in the areas listed above. There were 13 children and 5 staff present. The inspector reviewed 4 children?s records and 4 staff records electronically. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas of the center outside shall be maintained in a safe condition.

There are areas on the playground in which the resilient surfacing has holes in it.
Plan of Correction: Maintenance will be notified of the resilient surfacing holes.
Standard 8VAC20-780-570-E
Infant bottles shall be dated and labeled with the child's name.

A bottle for an infant was not labeled with the child's name or dated.
Plan of Correction: Bottles will be labeled and dated when there is more than one infant in care/bottle in use.
Standard 8VAC20-780-60-A
The center shall keep documentation of viewing proof of the child's identity and age within 7 days of enrollment.

Child #3 did not have documentation of viewing of proof of the child's identity and age within 7 days enrollment.
Plan of Correction: The proof of ID and age were received from the parent.
