Inspector Notes

An unannounced, on-site code compliance monitoring inspection was initiated on 09/09/2024 and completed on 09/09/2024. The on-site inspection began at 10:30 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 18 children present with four staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 22.1-289.031-A
A statement is to be posted to the face that the center is exempt from licensure in a visible location on the premises along with the qualifications of the personnel employed therein.

A statement was not posted per the director.
Plan of Correction: A letter of exemption and staff qualifications will be posted.
Standard 22.1-289.031-B-3
The center is to establish and implement a procedure in child a daily simple health screening and exclusion of sick children by a person trained to perform such screenings is conducted daily.

The director stated each staff member is responsible for conducting daily health screenings but no one has been formally trained.
Plan of Correction: The staff will be trained in daily health screening. Documentation of the training will be added to the staff files.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #1, employed for three years, did not have documentation of requesting a central registry check or sex offender search.
Staff #3, employed for one year, did not have documentation of requesting a central registry check or sex offender search.
Plan of Correction: The out-of-state background checks will be requested within 10 days from the inspection. The results will be forwarded to the inspector. In the future all background checks will be processed prior to employment.

The center must obtain the results of the search of the central registry within the first 30 days of employment for staff hired prior to 07/01/2024.

Staff #1, who had been employed for three years, did not have a central registry search.
Plan of Correction: The background checks will be processed within 10 days from the inspection. The results will be forwarded to the inspector. In the future all background checks will be processed prior to employment.
