Violations

2
Standard 8VAC20-800-320-B
Based on observation, child-resistant protective covers larger than 1-1/14 inches in diameter were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1. The electrical outlet near the patio door did not have a protective cover on one outlet.
Plan of Correction: We vacuum the carpet and forgot to put it back on. I'll do it now.
Violation was corrected during inspection.
Standard 8VAC20-800-780-B
Based on record review, annually, the provider did not review with the parent the emergency contact information as required to ensure the information is correct.
Evidence:
1. The record for child #1, #3, and #4 did not have documentation of an annual review of the emergency contact information. The last documented review was 05/01/2023.
Plan of Correction: I will contact the parent now and have them to review today upon pick-up.
