During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The most recent documented immunizations for child #4 (23 months) is from when the child was 12 months old.
Plan of Correction: Management has requested an updated immunization. Parent stated the child has an appointment on 6/19/2025. Management will utilize tracking sheet and monitor monthly for updates.
Standard 8VAC20-780-280-D
Cleaning and sanitizing materials shall not be located above or next to food, food equipment, utensils or single-service articles.

A spray bottle of a bleach cleaner was observed stored on a shelf with disposable drinking cups in the Preschool 1 classroom.
Plan of Correction: Cleaning supplies were removed from cabinet and [put] in the cabinet under the sink. Management has labelled cabinets to reflect food equipment and/or cleaning supplies on 6/2/2025.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented.

Child #2 has a diagnosed food allergy, per the childs physical exam. Child #2 was not included on the centers allergy list.
Plan of Correction: Moving forward management will update allergy list every time a new child enrolls with us that has an allergy and/or monthly.
Standard 8VAC20-780-60-A
Childrens records shall contain all required information.

Child #6s record does not contain documentation of an annual review of the childs emergency contact information. The most recent documented review of the childs record is dated 2/5/2024.
Plan of Correction: Management has schedule for annual reviews to take place for families at the beginning of the school year and calendar year.
Standard 8VAC20-780-60-A-8
The center shall obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #2 has a diagnosed food allergy, per the childs physical exam. The center has not obtained a written care plan.
Plan of Correction: Management has contacted the family and requested the required paperwork. Moving forward management will collect all required allergy paperwork at meet and greet before childs state date.
