During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/6/2026. The on-site inspection began at 12 PM and ended at 1:35 PM. The inspector reviewed compliance in the areas listed above. There were 61 children present and 13 staff. The inspector reviewed 6 children?s records and 3 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center shall obtain repeat background checks for each employee every 5 years.

1. Staff #3s most recent sworn disclosure statement is dated 1/24/2020. The background check has been expired for approximately 13 months.
2. Staff #3s most recent out-of-state sex offender background check is dated 9/29/2020. The background check has been expired for approximately 5 months.
Plan of Correction: The sex offender background check was completed during the inspection. The sworn disclosure statement will be completed on Monday when the staff person returns.
Standard 8VAC20-780-570-A
When children are being fed in a high chair, the safety strap shall be used.

Five children were observed eating lunch in high chairs in the infant classroom. Five out of five children were not buckled in their chair using the safety strap.
Plan of Correction: Staff will be reminded to buckle the children into the chairs and we'll complete spot checks to make sure it is being done.

Childrens records shall contain all required information.

Child #6 has a diagnosed food allergy. The childs allergy action plan was not signed by the childs physician.
Plan of Correction: The form was signed by the parents. We will ask the parents to have the form signed by the doctor next week.
