Inspector Notes

An unannounced investigation was conducted jointly with another local agency in response to a self reported incident related to children left alone and unsupervised in a classroom. The date of the incident was October 26, 2022. Interviews were conducted with staff. Staff and child records were reviewed. The schools supervision policies and procedures were also reviewed. The information gathered during this investigation supports the allegation. Violations have been cited related to this incident and are listed in the Violation Notice. Standard 8VAC20-780-340-A
Based on staff interviews, staff failed to ensure the care protection and guidance when staff are supervising children.

Evidence:
There were 2 staff member's supervising 11 children.
1. On 10/26/2022, Child A's class was preparing to go outside for a walk. The children were going to the bathroom and putting on their coats. The class left the room.
2. A few minutes later Child A entered Staff D's classroom (the classrooms are connected by a bathroom) and stated that his teachers/class had left without him.
3. Within five minutes Staff A entered Staff D's classroom and asked why Child A was in the room.
3. Staff D explained that Child A had been in the bathroom and that his class had gone outside without him.
4. Staff A took Child A back to her office.
5. Staff A stated that Staff B nor Staff C returned to the school looking for Child A.
6. Staff B and Staff C returned to the classroom at the end of their outdoor time.
7. Staff A returned Child A to the Classroom.
8. Staff A called Child Protective Services, the Licensing Inspector and the parents to report the incident.
Plan of Correction: -Reviewing the supervision of children policy
-Staff need to supervise all children with actual sight and sound at all times
-Teachers need to have proper communication during class time and transitioning time

-Will display headcount board at the door to enforce the headcount measure properly

Based on record review and staff interviews, the center failed to ensure that staff followed the center's own polices and procedures.

Evidence:
The Staff Handbook lists under Prohibited Practices - Lack of Supervision including diverted attention for frequent or extended periods leaving the children unsupervised or having a child unaccounted for.
Plan of Correction: -Staff were asked to review Staff Handbook

-Staff was retrained on the school policy on the supervision of children
