Inspector Notes
An unannounced, on-site renewal inspection was conducted 9/18/2024 from 1:00pm- 2:30pm. The inspector reviewed compliance in the areas listed above. There were 2 children present and 2 caregivers. The inspector reviewed 3 children?s records, 3 caregivers records, and 4 household members records onsite. This inspection included document review, interviews, observations, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today. A POC submitted after this date will not appear on the public website.
Violations
11Caregiver #1 had a sworn disclosure that was dated 9/1/2019, which expired 9/1/2024.
The family day home is required to obtain a central registry check for each caregiver within 30 days of employment.
Caregiver #2, hired in 2022, did not have central registry findings.
There was no physical examination on file for child #3, who has been care for over two years.
Assistants' and substitute providers' records shall contain the name, address, verification of age, job title, date of employment, and the name, address, and telephone number of a person to be notified in an emergency.
The record for Caregiver #1 and #2 did not have documentation of address, verification of age, job title, date of employment, and the contact information of a person to be notified in an emergency.
Household member #1 and #2 had a tuberculosis screening that expired in August 2023. A new screening had not been obtained.
The bathroom used for children contained Clorox wipes and laundry detergent in an unlocked cabinet, making them accessible to children.
The provider stated attendance has not been recorded for several months.
A child's record shall contain the name, address, and telephone number of each custodial parent's place of employment and the name, address, and telephone number of two designated persons to contact in case of an emergency.
The records for child #1, #2, and #3 was missing employment information for both parents and the address for each emergency contact. Child #1 and #2 were present during the inspection and had been in care for over two years.
Before the child's first day of attendance, parents shall be provided, in writing, whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by the Code of Virginia.
Child #1 and #2 did not have documentation of liability of insurance.
Caregiver #2 did not know the address that was listed as the relocation site in the emergency preparedness plan.
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
An evacuation drill was not conducted for the month of August.
