Inspector Notes

A mandated monitoring inspection was conducted on 05/03/2024 from 8:00 am - 9:25 am. There were 5 children in care with 2 staff supervising. The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 2 staff records, and 3 household members were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-200-A
Based on review of staff records, the provider did not orient the assistant provider by the end of their first week of assuming job responsibilities.

Evidence:
Staff #2 (start date: 02/15/2024) did not have documentation of their orientation.
Plan of Correction: Provider will ensure that orientation is completed and documented within 7 days of start date.
Standard 8VAC20-800-240-A
Based on observation, the provider did not ensure areas outside were maintained in a clean, safe, and operable condition.

Evidence:
An outdoor play structure had rust and chipping paint.
Plan of Correction: Provider will repair and paint the area that was rusted.
Standard 8VAC20-800-250-E
Based on observation, the provider did not ensure that latex balloons were inaccessible to children.

Evidence:
Latex balloons were accessible to children.
Plan of Correction: Provider will ensure balloons are not accessible to children. Balloons were removed during inspection.
