Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/23/2024 and completed on 9/23/2024. The on-site inspection began at 12:05pm and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were eleven children present and two staff. The inspector reviewed five children?s records and two staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-800-60-B
A child's record shall contain telephone number of each custodial parent's place of
employment.

There were missing phone numbers for the custodial parent's place of employment for child #1, child #2, and child #4.

A child's record shall contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

There was no second emergency contact for child #2, and child #3.
Plan of Correction: The caregiver will obtain the missing work phone numbers and emergency contacts for the children.
