During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 3/3/2026. The on-site inspection began at 11:20am and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were twelve children present and two caregivers; the point total was twenty-six. The inspector reviewed five children?s records and five caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-B-3
The Family Day Home must have a current search of the central registry for household members. Household member #4, did not have a current central registry search and was expired by over one month.
Plan of Correction: The household member will obtain an updated central registry search.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current report of tuberculosis (TB) screening every two years from the date of the first screening. Household members #4 was overdue for a TB screening by over seven months.
Plan of Correction: The household member will get an updated TB screening this weekend.
Standard 8VAC20-800-200-A
The provider must orient assistants by the end of their first week of assuming job responsibilities. Assistant Caregiver #2 provided care for children for four weeks without receiving orientation.
Plan of Correction: The provider will give the assistant orientation training.
Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of 16 clock hours of training annually. Caregiver #1did not obtain any training hours in 2025.
Plan of Correction: The provider will make a plan to obtain 16 hours of training for this year and all future years.
Standard 8VAC20-800-60-B
Children's records are required to contain parental employment addresses and phone numbers. The record for Child #1 was missing the employment phone number for both parents.; the child was present during the inspection and had been in care for over a year. The record for Child #2 was missing the employment phone number for both parents.; the child was present during the inspection and had been in care for over one month. The record for Child #5 was missing the employment phone number for both parents.; the child was present during the inspection and had been in care for over six months.
Plan of Correction: The provider will obtain work phone numbers for the parents.

Documentation of additional immunizations is required once every six months for children under the age of two years. 'Child #5, three years old, last documented immunization was over 12 months ago.
Plan of Correction: The provider will obtain an updated immunization record from the parents.
