Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated and concluded on October 24, 2022. . There were 26 children present, ranging in ages from 2 months to age 4, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-340-D
Based on a review of 5 staff records and staff interviews, the licensee did not ensure in each grouping of children there is at least one staff member who meets the qualifications of a program leader or program director.

Evidence: Staff #2, designated as the 3 and 4 year old lead teacher does not meet the qualifications of program leader as confirmed by the Center Director. Staff #2 was the only staff observed supervising 6 children ages 3 and 4 during the inspection.
Plan of Correction: The staff member will take the additional hours needed to be lead qualified.
Standard 8VAC20-780-60-A
Based on review of 5 children?s records and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contained the required information.

Evidence: The following information is missing from 2 out of 5 of the children?s records reviewed.
1.Child #4 did not have the employment for both parents listed.
2.Child #2 did not have an immunization record that was signed by a physician, his designee, or an official of a local health department by their first date of attendance. The first date of attendance was 10-11-22.
Plan of Correction: The employment number has been obtained. 10-24-2022.
