Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 06/05/2024, between the hours of 10 am to 11:30 am. There were 8 children present (26 points), ranging from infant to preschool-age, with 2 caregiver supervising. The children were engaged in the following: toy play, diapering, bathroom breaks, hand washing, infant nap time; centers within the childcare area; outside play on the FDH playground, getting ready for lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 9 children files, 2 household records, and 2 staff files were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Based on review, 3 of 9 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (06/05/2024), the following child was without 2 emergency contacts with complete information: Child #2
2. The following children were missing the policy number for the listed medical insurance policy name on file: Child #2, Child # 5, and Child #9.

Based on review, 3 of 9 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for those under the age of two years.

On the date of inspection (06/05/2024), the following children did not have a current immunization page on file:
