Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/09/2025 and completed on 4/09/2025. The on-site inspection began at 11:50AM and ended at 12:30PM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 12. The inspector reviewed 2 children?s records and 3 caregiver/household member records.
This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-140-D
A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.

Caregiver A was not at the family day home. There was not a substitute provider record to show the time of arrivals and departures of each day the substitute provider worked.
Plan of Correction: We will print the form to record substitute logs, and start using it right away.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

The TB test was not updated 2 years after the previous screening. Caregiver A & Caregiver B's subsequent screenings were overdue by approximately 2 weeks.
Plan of Correction: We will get the screenings completed right away.
Standard 8VAC20-800-80-C
The provider shall document in the child's record the method of verification for the child's age and identity.

The provider did not have completed documentation, for proof of identification, for Child A or Child B.
Plan of Correction: We will get the information documented right away.
