Violations
2Evidence: The toilet seat and seat cover in the Explorers Room bathroom used by children showed cracks and flaking of the water resistant covering allowing liquids to soak into the underlying porous materials.
Evidence: Measurements taken around the swing and climbers showed between 4.5 and 5 inches of resilient surfacing when at least 6 inches of surfacing was required based on the height of the equipment. The program director acknowledged the need for additional resilient surfacing.
