An unannounced inspection was completed on 10/29/2024 with another state agency, in response to a complaint received by the licensing office on 10/28/2024 relating to an allegation of special care provisions and emergencies. This inspection included interviews and tour of the facility.
The preponderance of evidence gathered during the investigation did not support the allegation; therefore, the complaint is determined to be not valid.
The inspection findings were updated 01/17/2025 as a result of a second-step review and 12/11/2025 as a result of a final review.
