During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 03/03/2026 and completed on 03/09/2026, in response to a self-report received by the licensing office on 02/24/2026 relating to forbidden action. The inspector reviewed one child?s record and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/20/2026. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
The center did not follow the center?s Staff Prohibited Behaviors policy. The Staff Prohibited Behaviors? policy states that employees are not permitted to display any of the following behaviors: hitting, spanking (refers to striking a child with an open hand on the buttocks or extremities with the intention of modifying behavior without causing physical injury), shaking, slapping, twisting, pulling, squeezing or biting. On 02/23/2026, Staff #1 slapped Child A (age 3 years) on the arm.
Plan of Correction: Staff #1 immediately self-reported the incident to Leadership. We will continue to reinforce a culture of accountability by encouraging self-reporting of any concerns. Staff #1 was immediately removed from classroom duties and direct child contact pending review. The child?s well-being was promptly assessed, and, thankfully, no injury or further concerns were noted. We will continue to prioritize the health and safety of the children in our care. The child?s parent was notified of the incident and kept updated during our internal review. We will continue to reinforce our commitment to transparency with our families. The incident was documented and reported to appropriate authorities. Staff #1 received formal disciplinary action and their employment terminated. We will continue to implement our staff disciplinary policy. All staff will complete mandatory retraining on positive behavior guidance, child development, and staff prohibited behaviors. Staff meetings will include ongoing discussions and scenario-based guidance on appropriate child management strategies. Leadership will conduct increased classroom observations to ensure adherence to policies and appropriate interactions with children.
Standard 8VAC20-780-410-1
The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.
On 02/23/2026, Staff #1 slapped Child A (age 3 years) on the arm while supervising children in Classroom #1.
