Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 04/30/2025. The on-site inspection began at 10:00AM and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were seventeen children present with two staff. The inspector reviewed five children?s records and four staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

Each person in a supervisory position shall be certified by a practicing physician or physician assistant annually, to be free from any disability which would prevent him from caring for children under his supervision. The most recent health report for Staff #3 was overdue eight months.
Plan of Correction: Per the director: We will contact staff today and have staff update, Will add sticker to file to help remind of annual requirement.
