Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 5/30/2025. The inspection started at 11:55am and concluded at 12:25pm. There were 3 children present (including provider's own child) and 1 caregiver; the point total was 5. The inspector reviewed 2 children?s records and 2 caregiver/household members records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

A family day home is required to obtain a copy of the results of a search of the central registry background checks for applicant and adult household members every five years.

The most recent central registry searches were completed over five and a half years ago for vendor and household member.
Plan of Correction: The vendor has submitted background checks and is waiting for results.
Standard 8VAC20-790-230-A
Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized according to the requirements of the Code of Virginia and applicable State Board of Health regulations.

Child #1 and Child #2 records did not contain documentation of immunization.

Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.

Documentation and vendor statement that only 8 training hours have been obtained and the department's health and safety update course has not been completed in the past year.
