Based on record review and staff interviews, the center did not inform the department's representative within two business days, of an injury to a child that occurred while under the supervision of the center, that required outside medical attention.
Evidence:
Child #1 was injured while in the center's care on 9/4/2024. The center was notified of the result of the injury on 9/5/2024, however the department was not notified until 9/9/2024.

Plan of Correction: Moving forward we will ensure that we notify licensing more quickly. We were waiting for the parent to submit the doctor's report.
