During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 04/29/2025, as a part of the licensure period. The on-site inspection began at 11:55 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 60 children present and 10 staff. The inspector reviewed 6 children?s records and 5 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/06/2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children, and shall have been completed within the last 30 days prior to employment.

Staff #4 had a TB screening that was dated over 90 days prior to their first date of employment. Staff #5 had a TB test dated over one week after their first date of employment.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

The changing tables in both the infants and toddler classrooms had locking mechanisms on their cabinet door to prevent access to cleaning materials, however the locking mechanisms were broken.

Staff and children's hands shall be washed with soap and running water after diapering.

A teacher in the two year old classroom was observed changing a child's diaper, and then allowing the child to go lay on their cot without washing their hands. The teacher was also observed tending to other duties without first washing their hands as well.
Plan of Correction: Implemented training for staff on proper diaper changing procedures on 4/30/2025
